Oakland Community Health Network

Oakland Community Health Network is contracted by the Michigan Department of Health and Human Services to lead a provider service network that assists approximately 27,000 Oakland County citizens at more than 300 service sites across the county. People who receive public behavioral health services through OCHN’s provider network include those who have an intellectual or developmental disability, mental health concerns, or substance use disorder. The organization’s goal is to ensure these individuals are aware of and have access to services and support that will improve their health and quality of life, as well as ensure their engagement in full community participation.

The Sanctuary is a short-term, safe shelter for youths ages 10 through 17 who are currently in crisis. Any youth in the community who has run away, is homeless, or is in an abusive, hostile, or intense situation at home can qualify for this voluntary program, but it does require parental consent and participation. The focus is on family reunification. Individuals stay at the Sanctuary for up to three weeks and receive around-the-clock supervision, intensive individual and family counseling, group counseling, and access to peer support. 

A Step Forward is a safe and home-like transitional living shelter where homeless youths (ages 16 through 17) may stay for up to two years while they attend school and maintain employment. Designed for youths seeking self-sufficiency, this program also offers counseling, job training, career development, educational assistance, financial subsidies and medical assistance.

The Graduated Apartment Program (GAP) provides independent living services for youths ages 18 through 24 years old who are experiencing homelessness. GAP offers up to 24 months of housing, case management, counseling, and educational and employment support to help people develop a greater level of self-sufficiency, interpersonal skills and housing readiness.
